With tadalafil, what I see most often at the pharmacy counter is patients underestimating how long it stays in the system. The upside is flexibility many people don’t have to “time it perfectly.” The downside is that some patients take another dose too soon or try to mix it with another ED medication the next day. My practical tip: because tadalafil can last up to about 36 hours, think of it as longer-acting follow the schedule your prescriber wrote and avoid “stacking” doses. If backache or muscle aches show up (a classic tadalafil complaint), hydration and a simple OTC pain reliever if safe for you often helps, but severe or persistent pain is worth checking in. And, just like all PDE5 inhibitors, I always screen for nitrates and “poppers” first if those are on board, tadalafil is not an option.
