Do not use if

  • You’ve had a serious allergy to metronidazole or other nitroimidazoles (e.g., tinidazole).
  • You consumed alcohol within the last 24 hours or plan to drink during therapy and for at least 3 days after—risk of severe reaction.
  • You’re taking disulfiram (avoid within 2 weeks).

Common side effects

Nausea, metallic taste, abdominal discomfort, headache, dark urine. Seek urgent care for severe allergic reaction, peripheral neuropathy (numbness/tingling), or severe CNS effects (seizures, confusion).

Antibiotic stewardship

What is Metronidazole?

Metronidazole (brand: Flagyl) is a nitroimidazole antibiotic/antiprotozoal used for certain susceptible infections. Available as immediate-release and extended-release tablets. Dispensed by Serv-U Pharmacy only with a valid prescription.

Uses
  • Bacterial vaginosis (ER tablet may be used when appropriate)
  • Trichomoniasis (treat sexual partners as directed by prescriber)
  • Giardiasis and amebiasis (per regimen)
  • Intra-abdominal and anaerobic infections (often with other agents)
  • Dental/orofacial anaerobic infections (as directed)
  • Note: For C. difficile colitis, other agents may be preferred—follow prescriber guidance.

How it’s taken (follow your prescription)

Take exactly as prescribed. Swallow tablets; ER tablets should be taken on an empty stomach (or as labeled). If you miss a dose, take it when remembered unless it’s almost time for the next—do not double up.

Warnings & interactions
  • Alcohol: Do not drink during therapy and for ≥3 days after finishing (risk of flushing, cramps, nausea, vomiting, tachycardia).
  • Disulfiram: Avoid use within 2 weeks of disulfiram (risk of psychotic reactions).
  • Warfarin and anticoagulants: May increase INR—monitor as directed.
  • Peripheral neuropathy/CNS effects: Report numbness, tingling, seizures, or confusion.
  • Hepatic impairment: Dose adjustments may be needed—inform your clinician.
  • Pregnancy/Breastfeeding: Discuss risks/benefits with your clinician for your specific situation.

Is Flagyl the same as metronidazole? Flagyl is a brand name for metronidazole. We typically dispense FDA-approved generics unless your prescription specifies otherwise.

Metronidazole is a workhorse against anaerobic bacteria and certain protozoa. It’s generally well tolerated and cost-effective, but counseling is critical: no alcohol during therapy and for several days after, watch for metallic taste and GI upset, and report any numbness or tingling. Used thoughtfully and for the right indication, it’s highly effective and helps us avoid broader-spectrum antibiotics when they aren’t needed.
